Portfolio Assigned days      2012-13 Candidates: Please fill out a COOP APPLICATION (lime green for 2012-13) in the Coop office or the Guidance office  BUT hand it in the Coop office only. (Sample Only-DO NOT PRINT) Recruitment for next year's jobs commences late February of THIS year. EMPLOYER 2. COOP OFFICE BY 8:00 A.M. IF THEY ARE GOING TO BE ABSENT FOR THE DAY Note - to be at work on non-school days, you need to make pre-arrangements with your Coop teacher and complete the neccessary paperwork (ie.-P.A. days, weekends, holidays...) Also, if you intend on working a full day(if you are half day Coop student) you must fill out and have signed a 'Request To Be Absent' form BEFORE you miss your regular classes Your Coop teachers for 2011-12 are:  Mr. McMurray       Mr. Prata           Ms. Grieve        Ms. Boyd             Durham Region Transit (Link to bus schedules)                 Calendars and other forms: Semester 2  - Coop calendar Work Extension Form (if you want to work weekends or hours not included on your contract) Request for Absence Form (if you want to stay longer at Coop and will miss one or more of your other classes)         COOP LOG SHEETS :   OPTIONS for filling out: 1. make a hard copy to fill out in pen or , 2. fill it out as a WORD document , have it signed by the employer and return it to the Coop office All assignments on the right-hand side of the timesheet must be completed for full marks.
